• Fax a copy of your prescription to - 24 Hours / 7 Days a Week to complete the online ordering process.

Option B - Order by Fax or Mail:

Step 1: Print the Order Form
(ensure that you select "Shrink to Fit" if you are printing the HTML order form)
Step 2: Complete and sign the Order Form.
Step 3: Fax to Toll Free , along with a copy of your original Prescription

Make sure your doctor’s license number and phone number is written on the prescription. If you do not have your original prescription have your doctor or pharmacy fax it to us. If you have no access to a fax, mail to:
